Rehabilitation Medicine Associates (RMA) is considered a specialty practice. You must have a referral from a physician to make an appointment in our clinic.
RMA is contracted with most of the health insurance companies in Oregon; this includes Worker Compensation and Motor-Vehicle Plans. However, prior to making an appointment at RMA, check with your insurance company to ensure that our specialists are on your preferred panel.
As a service to our patients we bill the insurance company for services received. We require your insurance card at each appointment and you are expected to make required co-pay payments at each visit. You will be billed for the remainder of your financial responsibility. Payment for services is required within 30 days.
If you are uninsured you will be considered a self-pay patient. As self-pay you will be required to make a deposit for services on your first visit to the clinic.
We do not hold accounts open pending court action involving litigation for motor vehicle or workers' compensation.
